About the role:

Opal Glenmere is currently seeking a Clinical Care Coordinator on a full time basis to act as a clinical expert, provide hands-on education to nursing team members and promote excellence in care. 

You will be required to support and contribute to quality improvement and demonstrate a high level of skill in clinical decision making, especially when it comes to problem identification interpreting clinical data, wound assessment and management. 

You will be expected to be able to assess for acute changes and respond to clinical deterioration and liaise with the Care manager, medical practitioners and allied health professionals. 

Our ideal candidates will demonstrate the following:
  • Bachelor of Nursing and ideally post graduate qualifications in dementia or mental health.
  • Current practicing registration as a Registered Nurse with AHPRA for at least 5 years.
  • Deep knowledge of the aged care industry, regulations and accreditation process.
  • Prior experience evaluating and reviewing practice standards and procedures, assessment tools and care plans in aged care. 
  • Understanding of Workplace Health and Safety framework and regulations.
  • Exceptional written and verbal communication and interpersonal skills including the ability to engage staff at all levels.
  • Well-developed report writing and analytical skills.
  • Proven coaching, problem solving and influencing skills.
  • Highly organised and previous experience with computerised care systems.
