Home Care Coordinator (Aged Care)

Excellent opportunity for a Home Care Coordinator with experience to join a values driven Aged Care organisation. You will be responsible for providing support and advice to the clients and their families to ensure timely admission into the service. You will oversee all arrangements of home assessments, ensure compliance, general operational duties & management of the staff. 

You will have the benefit of working alongside the executive team to spearhead this new community offering to their aged care network. This exciting new venture will allow the successful Home Care Coordinator to grow and develop within this business. Excellent career opportunities on offer  

To be successful in this role you will have: 
  • Extensive Home Care experience
  • Hands on experience of building a client base
  • Demonstrated experience in coordinating a flexible community team.
  • Knowledge of community Aged Care system.
  • Leadership, coaching and recruitment experience a must! 
  • Experience managing home care packages, case management, working with clients & developing Care plans. 
  • Valid NSW driving licence
