Experienced Aged Care Case Manager wanted to work initially 3 days per week, to assist in Independent living, South East Melbourne VIC

The Role:

The experienced Case Manager will be responsible for providing excellent service to internal and external customers as well as assisting with the effective operation of the home services.

Reporting directly to the General Manager, you will aim to maintain a high level of service efficiency. Your highly developed  assessment and referral skills, ability to plan, provide and evaluate services will assist in ensuring that the emotional and physical needs of the residents are met.

  • Undertaking comprehensive care assessments/reassessments in consultation with consumers & advocates
  • Developing comprehensive individualized partnership plans and consumer budgets that address consumer goals and assessed needs
  • Ensuring that all consumers are offered a Service Agreement
  • Negotiating a co-contribution by consumers, taking into account both legislated restrictions w and the consumer's capacity to contribute
  • Undertaking occupational health and safety assessments of consumer's homes, prior to the delivery of any services
  • Assisting with monitoring of the service performance of any external service providers
  • Assisting with preparation of annual service budgets and monthly variance reports
  • Ensuring a level of service delivered operates within the approved budget
  • Diploma  of Community Services Specialising in Case Management 
  • Relevant experience in planning, coordinating and delivering services to older people
  • Current and valid Australian National Police Check
  • Great customer service and interpersonal skills 
  • Ability to work with people from diverse socio-economic and  ethnic backgrounds 
  • Ability to gain cooperation from consumers and carers
  • Negotiation and advocacy skills
  • Ability to liaise with counterparts in other organisation to discuss specialist matters
  • Ability to work as a member of a team 
  • Demonstrated ability to work autonomously and as part of a multidisciplinary team.
