NDIS Support Coordinator    
In response to the high demand for Support Coordination services in South Australia, South East QLD, the Hills, Blacktown, Hunter Valley and Central Coast regions of NSW, we are recruiting for Support Coordinators in these areas.

About Support Coordination 

NDIS funded Support Coordination is the key to linking people with disabilities to their local communities and supports to live happy, healthy lives. Key functions being: 
  • Taking a holistic view by working effectively with the participant to coordinate, implement, and maximise on identified NDIS plan goals. 
  • Recognising issues/concerns and, where necessary, preparing reports to the Quality and Safeguards Commission. 
  • Identifying, collating and submitting requests for Change of Circumstances and/or Review of a Reviewable Decision. 
  • Build and maintain pro-active relationships with NDIS registered, non-registered providers and mainstream community-based resources ensuring participants are receiving qualitative value for money, from their chosen support. 
  • Undertaking regular training, continuous improvement, and staying abreast of key industry changes.   
Who are we looking for?
  • People that have extensive experience supporting individuals; that require complex intellectual, physical, psychosocial and behavioural support
  • People that have the ability to work autonomously and 'think outside the box' 
Essential  
  • 2-3+ years' experience working with people living with disabilities and complex needs 
  • Highly effective interpersonal skills with ability to overcome communication barriers 
  • Experience working with people from diverse backgrounds 
  • Ability to travel throughout the community and work from home 
  • Knowledge of the NDIS and NDIA 
  • Valid Driver's license and Comprehensively Insured Motor Vehicle 
  • Full Australian working rights 
  • Australia Business Number 
  • Valid Police Clearance 
  • Relevant states working with children's check  
Desirable  
  • Experience working as a Support Coordinator with people with severe disabilities & complex needs 
  • 5+ years' experience working within the disability or human services sector in a frontline role  
  • Tertiary qualifications in Disability, Human Services or Social Care

  • Extensive experience in Mental Health or Disabilities support
  • Relevant qualifications in social or community services 
  • A demonstrated flexible, people-centred approach to problem-solving and daily work   
  • Extensive knowledge of the local community, providers, services and community facilities   
  • A proven ability to work independently,self-direct and take initiative

Please note this role is an independent contract position, you will be required to manage all aspects of superannuation and tax requirements, etc. independently as any sole trader or small business would.
