Therapeutic Case Manager - Preservation and Restoration


Hold a caseload of 5, working with children ages 0-15 to provide therapeutic case management and support to families for up to 6 months.

Working under the Intensive Family Preservation model with voluntary families for up to 6 months you will provide specialist casework and therapeutic intervention for children ages 0-15.

This role will involve;
  • Holding a caseload of 5 with weekly visits occurring between 1-3 times a week
  • Development of case plans for children/young people and their families to provide ongoing therapeutic intervention and case management
  • Undertake assessments to identify children and family’s needs
  • Assist in the transition and placement of children/young people entering and exiting temporary and permanent care
  • Develop restoration plans or transition a child/young person into kinship care
  • Provide support and oversight to carers, as well as conducting annual reviews of carers
  • Maintain records for reporting and sharing of information
A successful candidate will like working in a fast-paced environment providing daily crisis support to clients.

The requirements for this role include;
  • A minimum of a bachelor’s degree in a relevant discipline such as Social Work, Psychology or Family Therapy
  • Extensive experience working with and providing quality assessment and case management services to children young people in OOHC
  • Demonstrated understanding of the principles of contemporary case management practice
  • Skilled in conflict resolution and mediation skills and excellent interpersonal and teamwork skills
  • Well developed oral and written communication skills
  • Sound computer literacy
