Community Support Workers required to support the elderly and people with disabilities so they can remain in their home. MIn Cert III required.

Support Workers
 
Due to ongoing growth our client is seeking newly qualified and experienced Support Workers to provide their clients with day to day support services ensuring they can live safely and independently within their homes.
 
Their clients include the Elderly and people living with disabilities and they offer a range of services including personal care, medication prompting, transport assistance, respite services and social programs.
 
As a caring and compassionate Support Worker, your duties will include:
  • Providing support to customers with personal care, living skills, transport, social activities, medication and other support as required
  • Providing a person centred approach to help customers achieve their goals and aspirations
  • Contributing information for the development of Customer support plans
  • Keeping customer records up to date on a daily basis
  • Developing and maintain professional relationships and networks with the customers, their families, and key stakeholders
  • With support & guidance from the Team Leader, seek out innovative ways to enhance the customer journey
To be successful in this role you must have a caring and compassionate nature, have completed a Cert III in Individual Support or Home and Community Care and have a valid driver’s licence.
