About the Role:

Our HomeCare Planners are responsible for the planning and implementation of Community Models of Care for customers with complex clinical needs. HomeCare Planners assist our Local Area Manager in providing supportive supervision of staff to ensure customer-focused quality care and services. This position will be based in Pinjarra Hills and surrounding locations. The hours are 8 am - 4 pm, 3 days a week, and flexibility may be required. For training, with notice, you may be required to travel to other sites.
The role will include:

  • Lead case management for high-level support programs i.e. HomeCare Package Programs level 1 to 4 and other funding programs such as CHSP
  • Lead clinical governance and manage the quality management framework within HomeCare services to ensure care standards are met
  • Champion the consumer-directed model of care
  • Build and maintain strong relationships with internal and external customers and local networks (e.g. Business partners, referrers, residents) to promote Bolton Clarke
  • Provide leadership and supervision for the day-to-day activities of the At Home Support team.
  • Provide direct and indirect clinical care to customers within scope of practice
What we need from you:
  • Current Nursing Registration with APHRA as a Registered Nurse
  • Relevant understanding and experience in the delivery of health services as well as complex case management
  • Demonstrated ability to lead a team to provide positive customer outcomes
  • Solid understanding of the Aged Care Sector and Aged Care funding models i.e -HomeCare Packages and program guidelines and service requirements
  • Federal Police Clearance
  • International Police Check if you have lived overseas in the last 10 years.
  • Current working rights in Australia
  • QLD Drivers Licence
  • Fully registered and Comprehensively insurance Vehicle
