Your Role: Based in our offices in Parramatta, NSW, Bolton Clarke are seeking a proactive and highly organised Care Coordinator to help manage a team of community based Personal Care Workers and nurses. You will be responsible for ensuring that client care visit schedules are met on an ongoing basis by coordinating care information and staff and updating information in the Company’s client management system. Your highly developed skills in team co-ordination and communication will put you in good stead to deal with the range of staff, clients and stakeholders involved. This role will be offered on a permanent full time basis. Key Responsibilities:

  • Roster co-ordination to ensure appropriate staff are matched to client needs
  • Records management, including timesheet and staff leave management, data collection, achieving KPIs and continual quality management and improvement.
  • Professional liaison with clients and external providers
  • Co-ordination of a community based employee needs
To successfully fill this role, you will have:

  • Experience in staff coordination or rostering in the Community Services sector
  • Experience of inbound and outbound call centre environment
  • Excellent time management and problem solving skills
  • Strong administration skills and computer literacy
  • Strong organisational and time management skills
  • Ability to work both independently and as part of a team
  • Outstanding written and verbal communication skills
  • Focus on providing high quality customer service
  • Experience with sophisticated rostering and client management systems is essential (ideally TCM)
