Located in Caboolture, Queensland, Fernhill is a caring community with a large variety of garden areas, communal activity rooms and spaces to share time with friends and family. This is a Permanent Part time opportunity working 20 hours per week.

As Clinical Coordinator, you will ensure our aged care clients and clinical employees are supported.

Combining case management, clinical governance, ACFI, rostering, and staff management, the role of Clinical Coordinator is essential to ensure the leadership and coordination of all aspects of clinical care at our Fernhill Retirement Community.  

Your role
  • Provide clinical leadership and supervision of care teams including Nurses, Personal Carers & Allied Health staff 
  • Develop care plans in consultation with the resident, family/carer and other members of the care team
  • Promote a culture of service delivery that aims to improve the lifestyle experiences of residents
  • Maintain an information system that supports and enhances the provision of accurate documention relating to the resident
What do we need from you?
  • Current Division 1 Registered Nurse registration with AHPRA
  • Minimum 2+ years’ experience as a Registered Nurse
  • Demonstrated experience in clinical governance, accreditation and ACFI
Essential Requirements
  • Current national police check or the ability to obtain one (essential)
