The role:
Southern Cross Care Queensland is currently sourcing an ACFI Coordinator to provide expert advice and support to Residential Aged Care staff around Funding evidence requirements, to ensure a high standard of documentation of assessed resident care needs to support funding claims.

You must hold current RN registration through AHPRA, have aged care experience, and a solid knowledge of and demonstrated working history with ACFI.
 
This role reports directly to our ACFI Lead and is also supported by an ACFI Officer.
  
The role is offered in a full time capacity and based at our Corporate Office in Upper Mount Gravatt.   With 11 Residential Care Facilities from Taroom (5 hours west of Maryborough) down to Edens Landing (southern suburbs of Brisbane), this role also requires frequent travel
  
To be successful in this role, you will possess:
  • Review and monitor care processes, tools and assessments to ensure they accurately capture resident care requirements and support funding claims
  • Assist in upskilling RAC staff in their knowledge of the funding process
  • Update ACFI related documentation and training material in line with legislative changes
  • Liaise with key stakeholders about ACFI submissions and funding opportunities
  • Support sites when they have ACFI validation visits
  • Engage with multiple disciplines to provide advice and assistance to meet ACFI requirements (e.g. Allied Health staff, GP’s).
  • Assist sites to develop a process for ongoing voluntary ACFI submission
  • Current AHPRA RN registration
