Family Therapist working with young children and their families and assist in the transition into temporary or permanent care.

Working 3-4 day per week collaboratively with a team of professionals to provide ongoing therapeutic intervention to young children's families.

Day to day responsibilities of the role:

  •  Develop case plans for children/young people and  their families in collaboration with stakeholders and provide ongoing therapeutic intervention and case management
  • Engage with children/young people and families and undertake assessments to identify their needs
  • Assist in the transition and placement of children /young people entering and exiting temporary and permanent care
  • Assist in the preservation of families to support families to enter the care system
  • Lead the achievement of permanency goals for families and children/young people (e.g., Adoption, Guardianship, Restoration, etc.) within case plan goal directives
  • Provide support and supervision for foster care and adoption placements
  • Provide support to children appearing in the NSW Children’s Court
  • Develop restoration plans or transition a child/young person to kinship care
  • Provide leaving care and aftercare support to children and young people as they prepare to and exit care, and aftercare up to the age of 25
  • Maintain records for reporting and sharing of information
  • Recruit, train and assess  foster carers and prospective adoptive parents
  • Provide counselling and mediation services

Skills and Experience:
 
  • Tertiary qualification
  • Extensive experience working with and providing quality assessment and case management services to children at-risk families or children/young people in out-of-home care
  • Demonstrated understanding of the principles of contemporary case management practice
  • Drivers license and ideally access to your own car
