Aboriginal health Practitioner/ Enrolled Nurse - Part-time to Fulltime (up to 37.5 hrs per week)
Working Location: Position available to be based in our Sonder Port Adelaide with the expectation to attend all sites including partner mainstream GP sites as required.

We are seeking an Aboriginal and/or Torres Strait Islander person who is an Aboriginal health Practitioner/Enrolled Nurse currently registered with Australian Health Practitioner Regulation Agency and have the knowledge and experience in assisting completion of the MBS 715 Aboriginal health assessment and working with Aboriginal and Torres Strait Islander communities/members.

The aboriginal health Practitioner/Enrolled Nurse will be responsible for working collaboratively with Aboriginal and Torres Strait Islander patients, general practitioners and practice staff to increase the uptake of MBS item 715 by providing cultural and clinical support, and mentoring to partner mainstream GP practices.
  
The Aboriginal Health Practitioner/Enrolled Nurse will work within a multidisciplinary team and is integral to ensuring cultural integrity and high standards of clinical care and client follow up.
  
The Aboriginal Health Practitioner/Enrolled Nurse will assist in providing comprehensive health screening, adult and child MBS 715 health checks in collaboration with the GP.
  
The successful applicant must have:
  • Broad clinical experience in primary health care including skills in planning, coordinating and decision making within a clinical setting especially in line with assisting in the completion of the MBS 715 Aboriginal health assessment.
  • Experience in working with and alongside Aboriginal communities and community members with the ability to provide culturally appropriate support and advocacy.
  • Ability to capture and share clinical information with relevant health care providers, including in electronic format.
  • A good understanding of the local health system including referral pathways.
