Coordinator of Supports

Do you have a passion for working with people with disabilities? This part time position coordinates support services within NDIS plans for people.

The Supports Coordinator will be responsible for connecting participants with a NDIS Plan and develop the most appropriate service delivery model around formal and informal services and supports in their community and based on the individual's needs. 

This position will be permanent full-time (38 hours Monday - Friday) and based from our SA office in Gawler. We are looking for someone who has a personal commitment to a person centered model of care. You will be provided a laptop and able to work a flexible schedule involving travel through the region to attend meetings with your client group.

Key Objectives
• Strengthen and enhance the participants abilities to coordinate supports and participate in the community and ensure services are delivered within their plan budget
•Assist the participants and their families in understanding and reviewing their NDIS Plans to ensure they are getting the maximum benefit and personalised care
• Build the capacity of the participants to achieve greater independence to self-direct services and supports in the longer term
• Assisting clients to understand the range of service options available
• Link clients to service providers, and support them to establish service agreements which meet their needs and goals
• Enabling clients to feel in control of their support services by coordinating service providers and supports; liaising with government agencies; and obtain quotes for services
• Seek to improve client outcomes by supporting requests for additional funding as and when required

Key Skills 
• Excellent visioning, planning and support co-ordination skills
• An understanding of the impact of disability and an ability to access specialist skills and resources to support clients with complex support needs
• Ability to interpret an NDIA plan and budget, and recommend service options within the guidelines of the plan and budget
• Ability to work with other team members in a positive and pro-active manner to facilitate service excellence, and exceed client expectation
• Strong communication, administration & computer skills
